A lot of people wait too long because they assume it is “just tension.” Sometimes it is. But sometimes there are mobility, strength, endurance, movement, or workload issues that keep it going.

What causes neck pain and headaches to keep coming back?

A lot of neck pain is not just about one tight muscle.

Usually, it is a mix of things like:

  • reduced neck and upper back mobility

  • poor tolerance to desk work, driving, training, or stress

  • sensitivity in the neck and nervous system

  • weakness or low endurance through the neck, shoulders, upper back, and trunk

  • headaches being influenced by neck stiffness, posture, breathing, or tension

  • repeatedly flaring things up without building enough capacity

That is why quick fixes often do not last.

The goal of treatment is not just to loosen something off for a day or two. It is to improve how your body handles daily life so symptoms settle and stay settled more consistently.

How we treat neck pain + headaches (what you can expect)

Your first appointment is focused on figuring out what is actually driving the problem.

We will go through:

1. Your symptom history

We will talk through:

• where you feel symptoms

• what they feel like

• what started them

• what makes them better or worse

• whether headaches seem to be linked to neck movement, posture, stress, sleep, or activity

2. Screening for anything that needs referral

If something suggests you should be referred out or medically investigated, I will tell you.

3. A physical assessment

Depending on your presentation, this may include:

• neck range of motion

• upper back and shoulder movement

• strength and endurance

• symptom behaviour

• posture and movement strategies

• testing for pain referral into the shoulder or arm

4. A treatment plan

You will leave knowing:

• what is most likely going on

• what to focus on right now

• what to temporarily modify

• what exercises or strategies to start with

• what the next steps are

The goal is to make things clear and practical.

How physiotherapy can help neck pain and headaches

Treatment depends on the person, but usually includes a mix of:

  • education and reassurance

  • mobility work where appropriate

  • exercise for the neck, shoulders, upper back, and trunk

  • strengthening and endurance work

  • movement retraining

  • headache and flare-up management strategies

  • hands-on treatment when helpful

  • dry needling / IMS when appropriate

Physiotherapy is not just about symptom relief. It is also about helping you move better, load better, and feel more confident getting back to work, exercise, and normal life.

Common types of neck pain and headaches I treat

People do not fit perfectly into labels, but some common patterns include:

Neck stiffness and tension

  • Often feels like the neck and upper traps are always tight, especially after desk work, stress, or poor sleep.

Tension headaches

  • Often described as pressure, tightness, or a band-like headache that seems connected to stress, posture, or muscle tension.

Cervicogenic headaches

  • Headaches that are strongly influenced by the neck. These often seem to start at the base of the skull or are aggravated by certain neck movements or sustained positions.

Pain into the shoulder blade or arm

  • Sometimes neck issues can also cause symptoms that refer into the upper back, shoulder blade, or arm.

Recurring flare-ups

  • Pain settles for a while, then comes back again with work, exercise, travel, sleep changes, or stress.

No matter the pattern, the treatment process is usually similar: reduce irritation, improve function, build tolerance, and reduce the chance of repeated flare-ups.

Neck pain treatment that goes beyond just temporary relief

A lot of people have already tried:

  • massage

  • stretching

  • cracking or popping their neck

  • resting

  • heat

  • random exercises online

Sometimes those things help temporarily. But if symptoms keep returning, there is usually more work to do.

My approach is to combine short-term symptom relief with longer-term rehab so you are not stuck relying on constant temporary fixes.

That may include improving:

  • neck mobility

  • upper back movement

  • shoulder function

  • postural endurance

  • exercise tolerance

  • work tolerance

  • confidence with movement

Neck Pain + Headache Physiotherapy FAQs

  • Yes, in many cases. Some headaches are strongly influenced by the neck, especially when they are associated with stiffness, posture, muscle tension, or certain neck movements.

  • Not always. Many cases of neck pain do not need imaging. If your symptoms suggest further medical follow-up is appropriate, I will let you know.

  • That can happen with some neck problems. Sometimes it is referred pain, and sometimes nerves are more involved. The assessment helps sort that out.

  • Very common. Long periods of sitting, screen time, and sustained posture can all contribute, especially if the neck is already sensitive or not tolerating load well.

  • Yes. Sleep position, pillow setup, stiffness, sensitivity, and general neck irritability can all play a role.

  • Yes, when appropriate. It can be a useful tool for some people, but it is not the whole plan.

  • It depends on how long symptoms have been there, how irritable they are, and what your goals are. Some people improve quickly. Others need a short rehab block to build longer-term change.
